I’ve decided to negate the sexism from the question, and give a female answer.
Yootha Joyce Needham (20 August 1927 – 24 August 1980), known as Yootha Joyce, was an English actress best known for playing Mildred Roper opposite Brian Murphy who player George Roper in the British sitcom Man About the House.
Man about the House was the inspiration for the American sitcom “Three’s Company”
Joyce died in hospital of liver failure four days after her 53rd birthday on 24 August 1980. Her co-star and good friend Brian Murphy was with her. After she died, it was discovered she had been drinking more than 1/2 a bottle of brandy a day and sometimes much more.
It’s timely to mention now, because Brian Murphy has just passed on – more than 40 years after she did. Brian was 92.
Here they both are in 1973
EDIT: It’s sad to know that many other actors have also had alcohol problems. This is my answer, based on a single case, as the question asked. If you feel the need to tell the world about other actors, why not write your own answer?
